All enrollment-related forms are available in FACTS.  

We also want to make books more accessible and appealing by adding clear book displays throughout the library. Our goal is to purchase 25 displays at $159 each. Any amount you contribute will directly support putting great books into students’ hands and making our library a vibrant, welcoming place for all.

To access all re-enrollment and application forms:

• Log in to the FACTS Family Portal
• Our district code is SMCCS-TX
• Type in your username and password
• Click on the "Apply/Enroll" button in the left menu

Click the "Enrollment/Reenrollment" button to re-enroll a current student.

The Online Enrollment system will open with a link to the enrollment packet for your currently enrolled child/children. The online process should take approximately 5-10 minutes to complete. Your information will be saved so you can quit and come back later.

Please complete each section of the form. The last steps request payment of the enrollment fee and an "enrollment packet review." Please remember to submit the forms when prompted. Once the re-enrollment form is successfully submitted, a confirmation email will be sent.

To complete an application for a new student, click the "Application" button. This form must be completed for any child attending SMC for the first time. Please do NOT complete this form if your child is a current SMC student.

Please complete each section of the form. Once you have finished, you will be prompted to pay the application fee and submit it. Once the form is successfully submitted, you will receive a confirmation email.

Deadline

The deadline to re-enroll for discount fees is Friday, April 24, 2026. To complete the process, the re-enrollment form must be submitted and the payment made.

Registration Fees:

02/1/26 - 02/20/26:** $35 per student (with a $115 discount)
02/21/26 - 03/23/26:** $65 per student (with an $85 discount)
03/24/26 - 04/24/26:** $95 per student (with a $55 discount)
04/25/26 and after:** $150 per student

Re-Enrollment Criteria

The following criteria will be considered in deciding whether to accept or reject this application.

• All tuition and fees (including, but not exclusive to, library, after-school program, and textbooks) must be current at the time of application.
• Students have excessive tardies or absences. A principal meeting will be required for re-enrollment.
• Students must have displayed successful academic growth and appropriate behavior. This agreement may stipulate retention, summer school/tutorial programs, or specific behavior-modification procedures, and re-registration will be contingent upon compliance with it.
• The support and loyalty of students and parents are significant success factors for this school. All students and their parents must jointly agree to abide by the school board policy, administrative regulations, school rules and policies, and classroom rules and procedures.
